I have read and agree to the Facility Use Agreement, including the express release of claims arising from ordinary negligence to the extent permitted by Illinois law.I acknowledge the risks of participation and agree to comply with Facility rules. If signing for a minor, I am their parent or legal guardian and authorize their participation and emergency medical treatment as described in the Agreement.I understand that a responsible adult must remain physically present and actively supervise all minors throughout their time at the Facility, and that each participating minor must have parental authorization on file.
